Once the registration documents have been fully completed and submitted, please gather the following documents and bring them to your child’s school:
1. Proof of Residency - acceptable documentation includes the following:
- mortgage agreement/statement
- a deed
- a lease
- current utility billl
- a property tax bill
2. Photo Identification of Parent/Legal Guardian:
- Driver's License
- RealID
- Passport
3. Proof of Age - acceptable documentation includes the following:
- Birth Certificate
Replacement certificates for those born in Pennsylvania can be obtained by contacting the Division of Vital Records at: (866)712-8238, 555 Walnut Street, 1st Floor, Harrisburg, PA 17101
- Notarized copy of Birth Certificate
- Baptismal Certificate
- Passport
